Changing your name on your Statement of Academic Record
How do I change my name on my Statement of Academic Record?

If you need to change the way that your name is recorded, you will need to complete Change of Details form and include a full colour scan of the original document/s (at the original size), listed below.
Australian Citizens
Provide one of the following:
- Australian Name Change Certificate
- Australian Marriage Certificate
- Australian Passport
- Australian Birth Certificate
- Australian Citizenship Certificate
In addition, you must include:
- Evidence that your name has been updated on your USI (Unique Student Identifier) account
- Evidence that your name has been updated with the Australian Taxation Office (ATO).
New Zealand Citizens, Permanent Residents or Humanitarian Visa Holders
Provide one of the following:
- New Passport
- New ImmiCard
In addition, include:
Evidence that your name has been updated on your USI (Unique Student Identifier) account.
International Students
Provide:
- New Passport
and
Evidence that your name has been updated on your USI (Unique Student Identifier) account.
